THE RIGHT CARE IN THE RIGHT PLACE
Supporting aged care residents with expert healthcare options.
AVOIDING UNNECESSARY HOSPITAL TRANSFERS
The hospital is not always the best place for you or someone you care about.
Emergency departments are busy places where patients are prioritised based on urgency. For many aged care residents, staying in the comfort of their home with familiar staff providing expert care is often the best choice.
Residential Aged Care Homes in our region have specialist support systems in place to manage many clinical needs without transferring a resident to the hospital.
This helps to:
- Reduce unnecessary stress and disruption
- Maintain a familiar and comfortable environment
- Provide timely care through expert healthcare professionals
EXPERT IN-HOME CARE 24/7
Residential Aged Care Home providing round-the-clock care.
Comprehensive clinical care includes:
- Ongoing care for long-term health conditions
- Doctor check-ups in person or by video or phone
- Managing medications
- Specialist doctor visits
- Care planning meetings
- Planning for future healthcare needs (Advance Care Planning)
- End of life and palliative care
Led by experienced personal carers and registered nurses, and supported by other medical professionals like GPs. Wherever possible, Residential Aged Care Homes focus on caring for residents in their home, aligned with their health needs and preferences.
Aged Care staff will always contact family and carers if a resident’s health changes.
PLAN AHEAD
FOR WHEN IT
MATTERS MOST
- Talk about care early – discuss hospital visits and treatment choices with family, GPs and aged care staff.
- Write down care wishes – completing an Advance Health Directive or Statement of Choices records these decisions.
- Take part in care planning meetings – residents, families and carers can help shape ongoing care.
- Ensure health records are up to date – setting up My Health Record helps doctors and nurses access important medical details.
Having clear conversations and plans in place can help ensure care decisions reflect personal wishes.
IF HEALTH
NEEDS CHANGE!
Residential Aged Care Homes have a trained team that can manage many health issues on-site, with access to expert medical support—reducing the need for hospital visits.
If a resident becomes unwell, staff will check their condition and call in the right medical team.
When health needs change, Aged Care staff act quickly to arrange the right care.
This may include:
- General practitioners (GPs)
- Allied health professionals, such as physiotherapists
- GERI-RaSS (a hospital team that supports and visits aged care homes)
WORKING TOGETHER TO PROVIDE THE BEST CARE
